HOW TO SELF-REFER FOR THERAPY?

It is important that the initiative for making an appointment and deciding to see a Wellspring therapist comes from you. This can be done by phone, email or letter, when you will be sent information and a self-referral form, or by downloading our self-referral form.

When you have completed the form, we would be grateful if you would enclose a cheque or postal order for £10 (£5 if you are in receipt of benefit), made out to Wellspring. This administration charge is necessary due to ever-rising costs and the difficult financial situation facing most charities at the present time.

The self-referral form will be treated in strictest confidence.
