SELF-REFERRAL GUIDANCE

As part of our application process we require you to complete our "Self-Referral" and "Data Protection" forms.

Could you please also write a letter outlining the difficulties and issues you would like to discuss, to help us find a therapist suited to your needs. This can be as short or as long as you would like and need not be more than a line or two.

The application forms and your letter will be treated in strictest confidence.

When you have completed the application forms and your letter, we would be grateful if you would enclose a cheque or postal order for £10 (£5 if you are in receipt of benefit), made out to Wellspring. This administration charge is necessary due to ever-rising costs and the difficult financial situation facing most charities at the present time.
